WHAT IS THE ACTIVE AVONDALE SCHOLARSHIP?

The Active Avondale Scholarship Program assists eligible Avondale residents and their families by providing financial assistance to each applicant. The scholarship program applies to Avondale Parks & Recreation Department activities and programs which require a registration fee.

PROGRAM GUIDELINES

• Scholarship assistance is available only for Avondale Residents.
• Applicants must be Head of Household or 18+.
• Scholarships are available per fiscal year (July 1, 2025, to June 30, 2026).
• Scholarships are dependent upon available donor funds and can be terminated or suspended at any time.
• Scholarship financial assistance is $200 maximum, per family per fiscal year.
• Family financial assistance is determined based on Federal Poverty Level Guidelines (see chart).
• All financial documentation and a completed activity registration form must be submitted with the Program Fee Assistance Application. If approved, only an Activity Registration Form is required for each requested, specific class/program stating that your Scholarship Application was approved.
• Allow three weeks for application review and processing.

FINANCIAL DOCUMENTATION REQUIRED WITH APPLICATION

Bring in any two of the following with a photo identification card: (Copies Only; DO NOT submit original documents)

• Most recent Federal Tax Return for individuals in household receiving income
• Most recent paycheck or unemployment check stub
• Most recent Social Security or Social Security Disability Award letter(s)
• Supplemental Nutrition Assistance Program (SNAP) or Women, Infants, Children (WIC) verifications

INSTRUCTIONS

• Please review your application to make sure it is legible and complete.
• Include all required copies of financial documentation. (Do not submit originals. The City of Avondale is not responsible for any original financial documents submitted).
• Include a completed Activity Registration Form & Documents.
• Return your completed application and all other documents, in person at Avondale City Hall, via email at ActiveAvondale@avondaleaz.gov, or mailed in to:
