Group Home Registration Form

In certain residential zoning districts, the City of Avondale requires registration of group housing that is licensed by the State of Arizona and provides supervised care to individuals who are living together by reason of age or disability. There is a non-refundable registration fee of $626. 

Each group housing type is only allowed under certain conditions.  Please refer to Avondale Municipal Code, Chapter 28, Section 28-15 for a definition of each housing type and Section 28-33, uses permitted with conditions.

Other care facilities located in residential zoning districts must comply with standards and State license requirements, and may need approval of a Conditional Use Permit, but they are not currently required to register with the City of Avondale.  Please refer to Avondale Municipal Code, Chapter 28, Section 28-33, uses permitted with conditions and Section 28-34, uses permitted with a conditional use permit.
